Roller Skating Liability Waiver

I. Introduction

This Standard Liability Waiver ("Waiver") is designed to protect the interests of [805 Cheap Skate] ("Company") and participants ("Participant") in any activities or events organized by the Company. By signing this Waiver, the Participant acknowledges and agrees to the terms and conditions set forth below.

II. Assumption of Risk

The Participant acknowledges and agrees to the following: Roller Skating and Premesses

• The activities involve inherent risks, including but not limited to physical injury, property damage, or other losses.

• The Participant voluntarily assumes all risks associated with participating in the activities, whether known or unknown.

• The Participant is responsible for their own safety and well-being during the activities.

III. Release of Liability

By signing this Waiver, the Participant agrees to the following:

• Release, discharge, and hold harmless the Company, its officers, employees, agents, and volunteers from any and all claims, liabilities, or demands arising from the Participant's participation in the activities.

• Indemnify and defend the Company from any claims or lawsuits brought by third parties arising out of the Participant's actions during the activities.

• Waive any right to sue the Company for any personal injuries, property damage, or other losses incurred while participating in the activities.

IV. Medical Treatment

The Participant acknowledges and agrees to the following:

• The Company is not responsible for providing any medical treatment during the activities.

• In the event of an emergency, the Participant consents to receiving medical treatment and agrees to be responsible for any costs associated with such treatment.

V. Emergency Contact

The Participant agrees to provide the following information:

• An emergency contact name and phone number.

• Any relevant medical conditions or allergies that may affect the Participant’s safety during the activities.

VI. Governing Law

This Waiver shall be governed by and construed in accordance with the laws of the state/country in which the Company is located. Any disputes arising from this Waiver shall be resolved in the courts of that state/country.

VII. Miscellaneous

The Participant agrees to the following:

• This Waiver constitutes the entire agreement between the Participant and the Company and supersedes any prior agreements, understandings, or representations.

• If any provision of this Waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.

• By signing this Waiver, the Participant acknowledges that they have read, understood, and voluntarily agreed to its terms.

VIII. Signature

By signing below, the Participant affirms that they have read, understood, and agree to all terms and conditions of this Waiver.
